What are Moodle activities?
An activity is a general name for a group of features in a Moodle course. Usually an activity is something that a student will do that interacts with other students and or the teacher.

In the Add an activity or resource window, select Questionnaire and then click Add. The Adding a new Questionnaire page will open. In the Name field, enter a name for the questionnaire (Required). In the Description field, enter the text you want students to see at the top of the questionnaire.
